What is HRT

Replacing what the ovaries stop making, in the forms your body recognizes.

Hormone replacement therapy restores estrogen, progesterone, and, where appropriate, testosterone to physiologic levels during perimenopause and menopause. Bioidentical formulations match the molecular structure of the hormones your body produced before the transition, which is why what we prescribe is also BHRT: the standard of care today.

Current specialty-society guidance is clear: for most healthy women starting within ten years of menopause, HRT has a favorable benefit-risk profile. The question of "should I be on this" deserves a real conversation with a provider, not a 90 second screening.

And if you're still cycling: perimenopause is when many women feel the worst, and treating those symptoms doesn't require waiting for menopause to be complete. Three doctors saying "your labs are normal" is not the same as feeling like yourself.

Symptom map

The symptoms HRT can actually help with.

Perimenopause and menopause produce symptoms across multiple systems, and most women have been told some version of "it's just stress" for at least one of them. Here's what's typically driven by the hormonal shift.

Vasomotor

Hot flashes & night sweats

The most recognized symptom, and the one therapy is most clearly effective for. Most women see a major reduction in frequency and severity.

Sleep & cognition

Insomnia, brain fog, memory

Often the symptom women care about most but mention last. Fragmented sleep compounds with direct cognitive effects of the hormonal shift.

Mood

Anxiety, irritability, low mood

Perimenopause is one of the highest-risk windows for new mood symptoms in a woman's life. Hormone therapy often helps where other approaches alone haven't.

Genitourinary

Dryness, painful sex, UTIs

Highly treatable, including with localized options that are appropriate even for many women who can't take systemic therapy.

Metabolic

Weight and body composition

The transition shifts fat toward the middle and reduces lean mass. The picture is generally better on therapy, especially paired with strength training and protein.

Desire

Low libido & arousal

Multifactorial: tissue, lubrication, and desire respond to different levers, including low-dose testosterone for women where appropriate.

Realistic expectations

What it can, and can't, do.

Setting expectations honestly is part of the work. Here's the straight version.

What HRT does well

  • Reduces hot flashes and night sweats substantially for most women
  • Improves sleep quality, often within the first month
  • Resolves genitourinary symptoms, including with localized options
  • Often improves mood, anxiety, and cognitive symptoms through the transition
  • Preserves bone density and reduces fracture risk
  • Has a favorable profile when started within ten years of menopause

What it can't do

  • Replace strength training, sleep, protein, or stress management
  • Cause significant weight loss on its own
  • Reverse changes long established before treatment
  • Fit every history: certain cancers, clotting disorders, and unexplained bleeding change the answer
  • Be prescribed responsibly without baseline labs and a real workup
  • Continue indefinitely without periodic reassessment

How it works

From consultation to plan, in real medicine.

HRT prescribing follows the same pattern as any thoughtful medical decision: proper workup first, then a plan based on what your labs, history, and symptoms actually show.

Day 1 · 45 min

Consultation

In person at our Houston clinic. We map your symptoms across every category, review your history, and discuss what therapy can and can't do.

Days 2 to 4

Lab work

Comprehensive baseline panel drawn in our office, plus a body composition scan, read against optimal ranges rather than just population averages.

By day 5

Protocol design

Your provider designs your protocol: route, components, and starting doses calibrated to your labs and symptom severity, with the reasoning explained.

Days 7 to 10

First prescription

Filled through a licensed pharmacy. Administration guidance and honest timelines: many symptoms improve within weeks, others take longer.

Ongoing

Monitor & adjust

Structured check-ins at 6 weeks, 3 months, 6 months, then annually. Doses adjusted to your actual response.

Before you book

Questions everyone asks.

How much does HRT cost in Houston?
The comprehensive consultation is $349, including the 40+ biomarker panel, body composition scan, and both provider visits. Medication and program costs are reviewed with you before anything begins. Many expenses are HSA/FSA eligible.
Isn't HRT supposed to be risky?
The 2002 findings that scared a generation of women were more nuanced than reported, and current specialty-society guidance recognizes a favorable benefit-risk profile for symptomatic women starting within ten years of menopause. We review your specific risk picture at your consult.
I'm in perimenopause but still cycling. Can I start now?
Often yes. Perimenopause is when many women experience the worst symptoms, and treating them doesn't require waiting for menopause to be complete. It takes more careful protocol design, and that's exactly what the consultation is for.
Is this bioidentical hormone therapy?
Yes. What we prescribe is bioidentical, and most of it is FDA-approved bioidentical, with compounded preparations used when there's a specific clinical reason. See our BHRT page for how we think about the distinction.
What about testosterone for women?
Where appropriate, low-dose testosterone at female-physiologic doses is part of the conversation, particularly for desire and energy that haven't responded to the rest of the picture. See our testosterone page.
