What is TRT

Restoring testosterone in physiologic doses, with the workup it deserves.

Testosterone replacement therapy restores testosterone to a healthy physiologic range in patients with confirmed deficiency. What separates good TRT from bad TRT isn't the medication. It's the workup, the dosing, and the monitoring.

The clinical standard for diagnosing testosterone deficiency requires at least two morning testosterone measurements showing low values, plus a consistent symptom picture. We follow that standard. We don't write prescriptions off a five minute online quiz, and we don't dose to supraphysiologic ranges to chase faster results. Most men do well in the upper half of the normal range, not above it.

For women: low-dose testosterone is the most consistently under-prescribed hormone in modern medicine. Symptoms like flat libido, low energy, and lost muscle tone often respond to careful female-physiologic dosing with proper labs and monitoring. Where it fits, we treat it as seriously as we treat men's therapy, and we discuss it as part of the broader hormone picture alongside HRT.

Symptoms & labs

What low testosterone actually looks like.

Low testosterone shows up across multiple systems, and the symptoms overlap with sleep problems, stress, and thyroid issues. That's why diagnosis takes both a clinical picture and lab confirmation, not a self-report quiz.

Energy & drive

Fatigue and lost motivation

The most common reason patients come in. Persistent low energy not explained by sleep, training, or workload, and a sense the drive you used to have isn't there.

Libido

Low desire, performance changes

Low testosterone is one cause among several. Vascular, psychological, sleep, and medication effects matter too. Treating the right cause matters more than treating the obvious one.

Body composition

Losing muscle, gaining middle

Training the same way that used to work and getting different results is consistent with low testosterone, and also with insulin resistance. Often it's both. The labs sort it out.

Mood & cognition

Low mood, brain fog, irritability

Low testosterone can present as low-grade depression that doesn't respond to standard treatment. It's real, it's measurable, and it deserves a careful differential.

Sleep & recovery

Poor sleep, slow bounce-back

Testosterone production is heavily sleep-dependent, and sleep apnea is common in men with low levels. We screen for it before prescribing, not after.

For women

Flat libido, energy, muscle tone

Low-dose testosterone for women, at female-physiologic doses, with the workup, dosing discipline, and monitoring the women's market rarely gets.

Realistic expectations

What it can, and can't, do.

Setting expectations honestly is part of the work. Here's the straight version.

What TRT does well

  • Restores energy, motivation, and mental edge in most properly diagnosed patients
  • Improves libido and sexual function when low testosterone is the actual cause
  • Increases lean mass and decreases fat mass when paired with resistance training
  • Often improves mood and cognitive symptoms
  • Has favorable cardiovascular safety in properly screened men at physiologic doses
  • Improves bone density and metabolic markers over time

What it can't do

  • Replace training, sleep, nutrition, or stress management
  • Be started casually by men trying to conceive: it suppresses fertility, and we discuss fertility-preserving alternatives first
  • Fit patients with certain untreated conditions found on screening
  • Deliver "20-year-old you" results: supraphysiologic dosing creates real risk for marginal benefit
  • Be prescribed responsibly without confirmatory morning labs
  • Continue indefinitely without ongoing monitoring

How it works

From consultation to plan, in real medicine.

TRT prescribing requires a real workup and confirmatory labs, because dosing correctly depends on knowing where you actually are, and ruling out things that look like low testosterone but aren't.

Days 7 to 10

First prescription

Before you book

Questions everyone asks.

How much does TRT cost in Houston?
The comprehensive consultation is $349, including the 40+ biomarker panel, body composition scan, and both provider visits. Medication and ongoing program costs are reviewed with you at your visit before anything starts, with no surprises. Many expenses are HSA/FSA eligible.
How is The Tide different from an online low-T clinic?
Online clinics typically prescribe after a brief questionnaire and a single draw. We confirm deficiency with repeat morning labs, rule out treatable causes like sleep apnea and thyroid issues, dose to physiologic ranges, and monitor on a real schedule. The medicine is different.
Will TRT affect my fertility?
Exogenous testosterone suppresses your body's own production and can shut down sperm production. If fertility matters to you now or in the next few years, we discuss fertility-preserving alternatives and timing before anything is prescribed. That conversation is not optional here.
Do you treat women with testosterone?
Yes. Low-dose testosterone for women, at female-physiologic doses with careful monitoring, is part of our women's hormone practice. High-dose approaches and set-and-forget dosing are the main failure mode in the women's market, and we don't practice that way.
Do you offer pellets?
Pellets are one of several delivery options, and we offer them when they fit a specific patient, typically someone experienced and stable on therapy. Adjustable methods are usually the better starting point. See our hormone pellets page for the full conversation.

The next step

Start with a conversation, not a prescription.

A 45 minute consultation at our Houston clinic. We'll review your situation and decide together whether TRT fits, or whether the symptoms point somewhere else worth investigating first.
